Responsibilities
Essential Duties and Responsibilities (includes, but is not limited to):
- Provide direct patient care, including hygiene support, mobility assistance, and comfort measures.
- Monitor vital signs, document observations, and report changes in patient condition.
- Administer medications as prescribed and observe for reactions or side effects.
- Perform wound care, dressing changes, and assist with procedures within the state LPN scope.
- Collaborate with Rn's, physicians, and the care team to support individualized care plans.
- Assist with discharge planning and provide patient/family education on treatments and recovery.
- Follow infection control practices and maintain accurate patient records.

- Current and valid state license
- Current BLS certificate upon hire and maintain current
- IV certification upon hire (state-specific)
