Overview
We are seeking a dedicated and skilled Assessment Nurse to join our team. As an Assessment Nurse, you will play a vital role in evaluating patient needs and coordinating service care plans.
Responsibilities
- Conduct or review/ approve resident assessments; develop or review/approve resident service plans.
- Conduct regular quality assurance reviews.
- Conduct monthly audits of Medication Administration Records (MARs).
- Conduct and/or coordinate staff in-services and trainings, as requested
- Provide health counseling/education to residents as needed.
- Perform all other duties as assigned.
Qualifications
- Active RN License preferred or LPN License
