Job description:
We are seeking a compassionate and dedicated Home Health Nurse to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in their homes while promoting comfort, independence, and recovery.
Responsibilities:
- Deliver skilled nursing care during home visits
- Monitor patient health and update care plans
- Educate patients and caregivers
- Coordinate with healthcare providers
- Maintain accurate patient documentation
Qualifications:
- Active RN license
- CPR/BLS certification
- Strong clinical and communication skills
- Home health experience preferred
- Ability to travel to patient homes
- Knowledgeable in Data Soft Logic (DSL) EMR. If not, willing to be trained
