Social Work Assistant

Job Summary
We are seeking a dedicated and compassionate Social Work Assistant to join our dynamic team. Your energetic approach will help foster positive change, promote well-being, and empower geriatric individuals to overcome challenges. You will provide counseling to our residents throughout their stay in order to help them meet their psychosocial needs.

Requirements

• Bachelor of social work or related field
• Demonstrated experience or training in social work, behavioral health, or related fields.
• Knowledge of Medicare and Medicaid programs
• Excellent communication skills with the ability to build trust with residents and families
• Ability to handle sensitive situations with professionalism, transparency, and compassion.

Responsibilities

• Assist with the admission process
• Provide social work services to residents and families to assist with adjustment to placement in a nursing home
• Provide ongoing one on one visits with residents
• Complete social work assessments, social histories, establish long and short term goals, and implement social work interventions
• Work with an interdisciplinary team to complete and revise care plans and attend related meetings and family conferences
• Will act as a resident advocate and assist with resident council
• Works with outside agencies to identify community resources related to discharging residents
• Assists residents with understanding medical expenses and provides information regarding Medicare and Medicaid. May also assist with application process
• Assists with the discharge process

Pay: From $47,000.00 per year
