St. Johnsbury Center for Living & Rehabilitation is seeking an energetic, motivated Director of Rehabilitation to lead our therapy team in a Skilled Nursing and Long-Term Care setting. This is an exciting opportunity for a licensed PTA or COTA who is ready to combine clinical expertise with leadership and operational oversight.

What You’ll Do:
Lead and manage the daily operations of the rehab department
Support, mentor, and develop PTAs, COTAs, PTs, OTs, and SLPs
Ensure compliance with state, federal, and company regulations
Oversee scheduling, productivity, and staffing coordination
Monitor clinical outcomes and quality measures
Collaborate with facility leadership and the interdisciplinary team
Oversee accurate and timely documentation in Net Health and PCC
What We’re Looking For:
Active Vermont PTA or COTA license
Previous SNF/LTC experience required
Prior leadership or supervisory experience preferred
Strong organizational and communication skills
Knowledge of rehab operations, Medicare guidelines, and case management
Positive, solution-focused mindset
