What are we looking for?
We are looking to add a Medical Appointment Specialist to our team. As a Medical Appointment Specialist, you provide support to individuals with disabilities in receiving medical care by scheduling, transporting and providing feedback after all medical appointments; assists with review of documentation related to individuals served; performs other job related duties as assigned.
Your overall essential responsibilities will be the following:
- Responsible for scheduling medical appointments for individuals in the Community Living Services program, providing transportation to/from all medical appointments, sharing information with physicians at appointments to ensure the health needs of the individual are met, and completing documentation after each appointment including any follow-up necessary, in the SJCS approved database and/or medication records.
- Responsible for reviewing daily notes of individuals served to ensure notes meet SJCS standard and if not, reporting findings to the Program Coordinator.
- Responsible for reviewing monthly documentation associated with individuals served and reporting findings, including errors/omissions, to the Program Coordinator.
- Observe and respond to individuals physical, social, behavioral and personal needs;
- Provide adequate supervision to individuals to facilitate their health and safety;
- Will:
- Be fully trained on individuals’ Individual Support Plan (ISP) and any special plans or therapy.
- Be fully trained on the individuals’ Behavior Support Plan (BSP) and report to supervisor as required for incidents.
- Implement and document on a daily basis the training outcomes and action steps in the ISP as required during the time period the support was delivered.
- Administer medication to individuals; ensure that administration is timely, in the right dosage and documented on the Medication Administration Record (MAR) as required.
- Transport individuals to and from appointments, employment and/or community sites;
- Ensure individuals are appropriately groomed for all activities, employment and
- appointments,
- Assist with the planning and implementation of daily activities;
- Prepare meals or assist with the preparation of meals in accordance with Individual Support Plan and any diet orders, adaptive equipment or mealtime plans;
- Assist with the daily household chores, to include but not limited to laundry, meal preparation and cleaning;
- Performs other job related duties as assigned.
Secondary Functions:
- Attend and participate in all relevant agency events, workshops, training and meetings;
- Performs other job related duties as assigned.
Required Knowledge, Skills, and Abilities:
- Ability to gain working knowledge of St. John’s Community Services policies and procedures;
- Ability to act as a representative of St. John’s Community Services to the public;
- Ability to effectively communicate and coordinate with other staff members and individuals from diverse backgrounds;
- Ability to pass a criminal background check;
- Knowledge of acceptable food preparation and safety practices and procedures; and
- Skilled in the operation of relevant computer systems, including hardware and software, and office machinery

- High school diploma or G.E.D. required
- Two or more years related experience working with persons with disabilities preferred; experience in community based residential programs a plus;
All SJCS employees must:
- Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.
- Meet SJCS and the funding authority requirement of criminal background and record checks.
- Valid state-issued driver's license with current driver's record in good standing

- Work environment involves risks or discomforts, which require special safety precautions e.g. safely operating a motor vehicle, exposure to aggressive human behavior.
