Responsibilities
The Continuing Care Assistant (CCA) supports case management and care transition services by coordinating post-acute care resources, assisting with discharge planning processes, and collecting and analyzing data related to patient transitions and continuing care services. This role collaborates with case management, social work, nursing, and providers to ensure efficient coordination of services and safe transitions across the continuum of care.
The Continuing Care Assistant also supports operational improvement by gathering, tracking, and analyzing data related to continuing care utilization, discharge outcomes, and post-acute placement trends. The CCA will develop statistical reports for upper level management, manage multiple projects, as well as meeting time constraints and expectations. Responsible for answering telephone, setting up meetings via outlook, entering insurance company denials and approvals in electronic utilization platform.
Qualifications
2–3 years of healthcare administrative, care coordination, or care management support experience preferred
Highly proficient in Microsoft Excel, PowerPoint, and Word. Project management experience.
Experience with data tracking, reporting, and healthcare analytics preferred
Familiarity with electronic medical records and healthcare data systems
Bachelor’s Degree in Health Administration, Healthcare Management, Public Health, Social Services, or related field preferred
