What are the responsibilities and job description for the Registered Dietitian position at St. Hope Foundation?
We are seeking a contractual or full-time REGISTERED DIETITIAN to become a part of our team! You will be responsible for conducting in-person or telehealth visits to assess patients' and clients' nutritional and health needs to assist them in living a healthy lifestyle or to achieve a specific health-related goal.
Responsibilities:
- Provides individual nutritional assessment plans to patient due to obesity or excessive weight.
- Identify client nutrition management needs for patients’ medical condition.
- Assist clients in developing a nutritional meal plan to better meet their dietary needs. Coordinate care with health care professional on the needs of the clients.
- Process referrals for access to pharmacy and other service providers.
- Provide nutrition consultation regarding promotion of behaviors for life-long good health and prevention and intervention of nutrition-related medical conditions.
- Perform essential work within Electronic Health Record system and program data entry systems with a high degree of accuracy and completeness.
- Communicate in a culturally competent and effective manner to clients/caregivers.
Qualifications
- Registered Dietitian (RD) in the State of Texas.
- A minimum of 1 year assessing nutritional needs and educating patients.
- Experience working in primary care or family practice setting.
- Ability to solve complex problems
- Good coordination skills
- Effective logic, reasoning, and critical thinking skills
- Detailed oriented and organized
- Great interpersonal, active listening, and communication skills
- Spanish-speaking a plus
Working Conditions
- The position occasionally requires sitting, standing, and bending. Occasional light lifting, up to 20 pounds or an equivalent weight may be required.
- Willing to travel to the Sugar Land and Greenspoint locations.
