What are the responsibilities and job description for the MDS Nurse (LPN or RN) position at St. Helena Parish Nursing Home?
We are seeking an experienced and compassionate RN to contribute in the crucial role of ensuring that our residents receive the appropriate and timely care necessary to maintain their health and well-being, while also ensuring compliance with relevant regulations and standards. Duties would include:
- Conduct comprehensive assessments of residents to identify their health conditions, functional abilities, cognitive status, and other relevant factors.
- Accurately and thoroughly document assessment data in the MDS (Minimum Data Set) forms, ensuring compliance with federal and state regulations.
- Collaborate with the interdisciplinary team to develop individualized care plans that address residents' needs and promote their well-being.
- Work effectively with other healthcare professionals, such as physicians, therapists, and social workers, to ensure coordinated and quality care.
- Stay current with MDS regulations, state and federal standards, and industry best practices.
- MDS experience preferred
- Current Louisiana RN license
- Maintaining accurate and timely documentation
- Supportive team environment
- Long-term care experience is a plus
