Skilled Nursing Admissions Coordinator (Weekend) - Part Time

Job Overview

• Admit residents from hospitals in accordance with established policies and procedures.
• Check patient referral portals consistently and communicate with Hospital Case Managers promptly regarding referrals.
• Effectively communicate with Administrator, Nursing staff, Admissions Director, and Community Liaison about prospective patients.
• Obtain the resident/guardian’s signature on all required permits, releases, authorizations, etc.
• Maintain a current listing of all residents, their level of care, and assigned room numbers.
• Notify nursing service when the resident has arrived at the facility to assure that a member of the nursing staff escorts the resident to his/her assigned room.
• Maintain a resident waiting list in accordance with established policies and procedures.
• Great customer service with a smile
• Provide facility tours to prospective residents and family members
• TEAMWORK & a positive can-do attitude

Education / Requirements

• High School or Equivalent
• Experience working in a long term care setting (Skilled Nursing Facility)
• Goal Driven
