Job Summary
Plans, organizes and implements clinical therapeutic services for Behavioral Health patients and families. Reports to Director of Clinical Services.
Essential Functions
Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
Interviews patients and family members to obtain information about home environment, family relationships, health history and patient’s personality traits.
Assists patients and families to understand, accept and follow medical recommendations.
Interprets nature of illness and prescribed course of treatment to patients and families. Communicates family issues to other staff members. Participates in planning conferences.
Assists in developing discharge and post-hospital care plans.
Collaborates with other hospital programs and community agencies with which patients and families are involved.
Prepares and maintains required documentation on each case.
Supports and interprets hospital’s Christian mission, philosophy and objective.
Maintains confidentiality of patients and other pertinent information.
Enhances professional growth and development through participation in educational programs, current literature, inservice meetings, and workshops. Attends meetings as required.
Represents behavioral health services to referral sources, community agencies, social services agencies and others as requested.
Completes psychosocial assessment
Qualifications
A master’s degree from an accredited school in counseling or related field is required. One to two years of experience in counseling is preferred. or Licensed Social Worker is required. Employee must demonstrates ability to recognize patients individual needs based on medical condition, age, limitations and planned procedures. This position requires the ability to maintain records and to use independent judgment, along with the knowledge of community resources and customer relations skills. Must demonstrate ability to recognize patients’ individual needs based on medical conditions, age (infants, pediatrics, adolescents, young adults, middle aged, and geriatric), limitations and planned procedures. This position has been determined to have a low risk of occupational exposure to blood-borne pathogens.
Working Environment
Ability to perform the following tasks if necessary:
Ability to participate in physical activity and to do bending, lifting, and/or standing on a regular basis.
Light lifting may be required.
Ability to work for extended period of time while sitting or standing.
