Role Description
We are seeking a full-time Children's Mental Health Case Manager to join our team in an on-site position located in Minnetonka and/or Minneapolis. The Case Manager will be responsible for coordinating and overseeing personalized mental health services for children and their families. Key responsibilities include conducting assessments, creating and implementing individualized service plans, collaborating with families and multidisciplinary teams, connecting clients to community resources, and ensuring timely documentation of services. The role aims to provide comprehensive support to children and families, promoting overall well-being and development.
This is a hybrid position as you may be at a home, in center, and/or out in the community representing St. David’s. You will have weekly contact with your direct supervisor and bi-weekly contact with the case management team for consultation.
Qualifications
- Driving is essential. Having a car and a valid driver's license is needed.
- Knowledge and experience in case management, including assessment, service planning, and coordination of care.
- Strong communication, interpersonal, and relationship-building skills for working with children, families, and diverse teams.
- Familiarity with mental health services and community-based resources and the ability to connect individuals to appropriate supports.
- Detail-oriented with the ability to complete accurate and timely documentation of services provided.
- Passion for supporting children and families, especially in mental health and developmental care.
- Minimum of a Bachelor’s degree in Social Work, Psychology, or a related field (Master’s preferred).
- Licensure or eligibility for licensure in social work (e.g., LSW, LGSW) or similar field in Minnesota is preferred.
- Experience working with children and individuals with mental health needs or developmental challenges is an asset.
