At St. Croix Hospice, the Licensed Practical Nurse plays an important role in delivering direct care to hospice patients under the guidance of a Registered Nurse. This position focuses on following individualized Plans of Care, administering treatments, and partnering with the interdisciplinary team to promote comfort and support each patient’s quality of life.

Qualifications:LPN Requirements & Core Responsibilities:
- Holds current Licensed Practical Nurse (LPN) license in state(s) of practice.
- Graduate of an accredited practical nurse or vocational nursing program required.
- Ability to pass background, consumer report and drug test.
- Provide direct patient care, follow the Plan of Care, and support end-of-life needs.
- Document all care accurately and promptly while educating patients and caregivers.
- Coordinate care by communicating patient needs to the interdisciplinary team.
- Manage pain and symptoms, use equipment safely, and maintain full HIPAA compliance.
- Must have reliable transportation, valid driver’s license, and proof of auto insurance

- Schedule: M-F 8:00am to 5:00pm, weekend visit coverage as needed, full-time position.
