Summary

The Care Manager functions as a facilitator of interdisciplinary collaboration across the care continuum.  The primary role of the Care Manager is to empower the patient/caregivers to assert a more active role during care transitions, and to develop lasting self-management skills.

Minimum Qualifications

1. Must possess a current license to practice professional nursing in the Commonwealth of Pennsylvania.
2. One year of clinical nursing experience.
3. Highly organized and well developed oral and written communication skills.
4. Appropriate clinical experience with a thorough knowledge of the scientific principles of nursing and professional nursing standards, developed skill in delivering nursing care, and the ability to adapt of individual needs.
5. A fundamental understanding of personal computers and familiarity with the Windows operating system.

Preferred Qualifications

1. Prior clinical experience in a related field and/or physician practice.
2. Bachelor’s degree in a health-related field.
