GENERAL SUMMARY:
The Crisis Stabilizer is responsible for providing support, crisis intervention, and stabilization to individuals in the Target Case Management (TCM) and/or Crisis Case Management (CCM) program. Through close collaboration with case management and the clinical team, they actively promote client wellness, assist in recovery & crisis plans, and foster community integration ensuring alignment with DHS requirements.
ESSENTIAL FUNCTIONS:
· Provides stabilization and support services for the client's well-being during periods of crisis.
· Assists clients in developing crisis & safety plans to ensure they have the supports needed during challenging situations.
· Supports the client’s treatment team in implementing the client's crisis & safety plan.
· Assists and provides guidance to clients to aid them through their recovery.
· Identifies and connects clients to local community resources.
· Communicate with applicable stakeholders as appropriate in a timely manner after each crisis incident.
· Provides assistance with the delivery of all aspects of support to the client.
· Participates in ongoing communication and collaboration with clients and other team members.
· Prepare, complete, and submit required items by due dates to include, but not limited to evaluation of services provided, documentation of all interventions, preventions, and stabilization contacts.
· Provide psychoeducation and support for independent activities of daily living for clients.
· Adheres to both St. Charles Youth & Family Services policies and procedures and Milwaukee County designated policies and procedures.
· Supports the goals of the agency and program mission/vision.
· Other duties as assigned.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
· Bachelor's degree in Human Services or a related field required.
· One year of experience working with the target population and/or providing crisis and mental health services.
· Strong organizational skills, ability to manage multiple complex tasks, strong professional communication and team work.
· Strong verbal and written communication skills, including the ability to perform record keeping, documentation, prepare formal reports using proper writing skills.
· Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
· Ability to perform computer tasks using MS Windows applications, Agency data base systems, etc.
· Valid Wisconsin driver’s license and personal liability insurance.
· Ability to use personal and agency vehicles for work related duties according to the agency’s insurance liability standards.
WORKING CONDITIONS:
· General office environment/multiple locations to include community sites and client’s homes.
· Travel approximately 50% using Agency’s vehicle(s).
