Family Medicine Physician – Bend East & Bend South

Your Schedule
- Full-time: 40 hours/week
- 36 hours of direct patient care
- Call rotation shared with clinic team (may include occasional evenings/weekends)
- On-site at either Bend East or Bend South clinic locations
Role Highlights
- Provide primary care for patients of all ages in an outpatient setting
- Deliver preventative care, chronic disease management, and acute visit services
- Collaborate with a multi-disciplinary team to coordinate care plans
- Refer patients to in-network specialists as needed
- Support the clinic's performance in population health metrics and value-based care goals
- May provide support or oversight for APPs as needed
Qualifications
Required:
- MD or DO from an accredited medical school
- Completion of a Family Medicine residency
- Board certified or board eligible in Family Medicine
- Licensed or eligible to practice in Oregon
- Active DEA and BLS certifications
Preferred:
- Experience in a Medical Home care model
- Familiarity with Epic EMR
- 2 years post-residency experience
