We are currently seeking a PER DIEM LPN to join our team, $23.00-$30.00 HOURLY based on experience.
Responsibilities:
This position performs tasks within the framework of case finding, health teaching, and health counseling under the supervision of a RN or MD. The provision of nursing services include the performance, direction, or supervision of health care tasks pursuant to a plan of care and consistent with an existing medical regimen.
· Provides ongoing, skilled nursing care appropriate to patients in the home setting as directed by the RN Case Manager
· Assists the home care patient to meet their planned nursing goals under the supervision of the RN Case Manager.
· Documents the patient’s care and clinical findings using agency established guidelines, including timeliness of documentation and in compliance with regulatory requirements.
· Reports clinical findings to RN Case Manager or Clinical Manager and documents the communication.
· When appropriate, orients and supervises Home Health Aides to the HHA Tasks.
Qualifications:
- Minimum of 1 year of experience in a hospital or acute care setting.
- Home Care experience is preferred.
- LPN licensure in New York State. Strong verbal, written, and computer skills required.
