The Healthcare Community Liaison plays a critical role in driving growth through strategic outreach,
community engagement, and intake support while representing the organization with
professionalism and integrity.
Key Responsibilities
Community Outreach & Relationship Building
● Develop and maintain strong referral relationships with hospitals, discharge planners,
assisted living facilities, skilled nursing facilities, senior centers, physicians' offices,
social workers, and other community partners
● Represent Aiding With Care at health fairs, community events, senior expos, and
networking meetings
● Conduct in-person and virtual outreach visits to introduce services and distribute
marketing materials
● Serve as a consistent point of contact for referral partners and community stakeholders
New Client Intake Support
● Support the intake process by responding to inquiries from prospective clients and
families
● Educate potential clients and referral sources on available services, eligibility, and next
steps
● Coordinate with internal administrative and clinical teams to ensure smooth intake
transitions
● Track referral sources and follow up on pending inquiries to support conversion
Marketing & Reporting Support
● Assist with community-based marketing efforts and grassroots outreach initiatives
● Maintain accurate documentation of outreach activities, referrals, and event participation
● Provide weekly activity reports and feedback on outreach effectiveness
● Identify new outreach opportunities and recommend strategies to increase client
engagement
Qualifications & Requirements
Required
● Reliable transportation and ability to travel locally to outreach locations and events
● Strong interpersonal, communication, and relationship-building skills
● Ability to represent the organization professionally in community and healthcare settings
● Organized, self-motivated, and comfortable working independently
● Availability to work flexible hours, including occasional evenings or weekends for events
Preferred
● Experience in healthcare, home care, social services, community outreach, or marketing
● Familiarity with referral-based service models and client intake processes
● Knowledge of local healthcare providers, senior services, or community resources
● Basic computer skills for reporting, email communication, and CRM or tracking tools
Performance Evaluation & Growth Opportunity
This position is part-time for the first 90 days, working 20–30 hours per week. Following a
successful 90-day evaluation based on outreach performance, referral growth, and intake
support outcomes, there is potential for transition to a full-time role.
