Normal hospital environment. Exposure to biological hazards. Frequent exposure to unpleasant odors. Close eye work. Hearing of normal and soft tones. Distinguish smells. Distinguish temperatures by touch and proximity. Operates FAX machines, copiers, printers, and uses computer terminal; Lifting up to 100 lbs. Carrying up to 40 lbs. Pushing/pulling up to 350 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching.

The Admit Discharge Nurse is responsible for the admission interview, medication reconciliation, and discharge process. The position must possess excellent communication skills and function at a high level as a member of the collaborative team. The role of this position is pivotal in coordinating the care of the patient pre and post discharge to facilitate an efficient and effective acute care stay, and coordinate accurate information to the next provider(s) of care to minimize readmissions. This position is required to utilize independent judgment.
