St. Bernard Physical Therapy department is comprised of Inpatient Acute Care and Outpatient Mix. This role is primarily for the Outpatient setting. Therapist are welcome to float to to acute care if interested.

Hours of Operation:
• Monday to Friday 8 am to 4:30 pm
• Optional to work weekend morning shift with pay differential.
Location:
• St. Bernard Ambulatory Care Center - 6307 S Stewart Ave, Chicago IL, 60621
Role:
• Outpatient Mix (primarily orthopedics)
Responsibilities:
• Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
• Provide physical therapy evaluation, treatment and follow up services to physician referred patient
• Collaborate with physicians to provide coordinated, holistic care
• 1 on 1 patient care (1 hour evaluation, 45 minute treatments, manageable caseload of
- 6-9 patients a day!
Minimum Qualifications:
• Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum
• Certification in Physical Therapy in valid state
• Possession of a current/valid American Heart
Association (AHA) Basic Life Support CPR
Certification
Pay and Benefits:
• $39/hr - $52/hr
