Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories to inform personalized care plans.
- Administer medications and injections accurately while adhering to aseptic techniques and safety protocols.
- Document patient information thoroughly using EMR (Electronic Medical Records) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Coordinate with physicians and specialists for diagnostic evaluations like laboratory testing, radiology (sonography), or medical imaging procedures.
- Assist in discharge planning, patient education on medication administration, health coaching, and post-acute care instructions.
- Demonstrate proficiency in specialized areas such as cardiology (cardiac catheterization), ophthalmology treatments (Botulinum toxin), infectious disease care, or geriatrics including memory care and dementia support.
Skills
- Extensive experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding for procedures and billing.
- Proficiency in performing advanced clinical skills including phlebotomy, specimen collection/processing, sterile processing techniques, and laboratory experience.
- Skilled in managing diverse patient populations including pediatrics, neonatal ventilator management, long-term care residents, hospice & palliative medicine patients or those with developmental disabilities. .
- Ability to handle complex cases involving infectious diseases, occupational health issues like Workers’ Compensation law cases or post-surgical recovery such as ostomy care or esthetic laser treatments.
- Excellent patient assessment skills with a focus on acute pain management.
- Knowledge of infection control practices and adherence to NCQA standards for quality healthcare delivery.
- Compassionate approach to working with vulnerable populations including seniors in assisted living or memory care facilities; experience with behavioral health needs is a plus.
