RNAC MDS Lead - RN - Registered Nurse

We are seeking to hire a RNAC MDS Lead - RN - Registered Nurse.
- $5,000 Sign-On Bonus*
- $44 to $48 (negotiable) per hour based on skills and experience.
- 7:30am to 4pm.
RNAC MDS Lead - RN - Registered Nurse Qualifications:
- Licensed as a Registered Nurse - RN in Pennsylvania.
- Minimum of 1 year of MDS Supervision experience.
- Minimum of 1 year of Long-Term Care experience.
- Minimum of 1 year of Care Plan experience.
RNAC MDS Lead - RN - Registered Nurse Introduction:
The RNAC Lead manages the accurate and timely completion of both the RAI and Care Management process from admission to discharge.
RNAC MDS Lead - RN - Registered Nurse Qualifications Job Summary:
- Manage MDS Team in all aspects of MDS process and administration.
- Complete all required sections of the MDS.
- Intensive focus on Care Plans: creation, documentation, monitoring and leading the Care Plan meetings.
- Ensure documentation is present in the medical record to support MDS coding.
- Work in collaboration with the IDT to assess the needs and formulate a plan of care for the residents.
- Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.
