Responsibilities
- Deliver comprehensive nursing care by performing physical examinations, vital sign assessments, and patient evaluations with precision and compassion.
- Administer medications and injections accurately following established protocols, including IV infusions and medication management.
- Assist with complex procedures such as catheterization, wound suturing, tube feeding, and sterile specimen collection while maintaining aseptic techniques.
- Monitor patients’ conditions closely, utilizing critical thinking to identify changes in health status and escalate concerns appropriately.
- Document all patient interactions thoroughly within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams—including physicians, case managers, and therapists—to develop personalized care plans for individuals with diverse needs such as dementia, Alzheimer's disease, or developmental disabilities.
- Provide education and support to patients and their families on health management strategies including pain control, medication administration, and discharge planning.
Requirements
- Valid license as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction.
- Proven experience in various healthcare settings such as hospitals (including ICU or Level I Trauma Centers), nursing homes, hospice care, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation.
- Strong knowledge of physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and medical terminology.
- Experience with specialized care areas including geriatrics, memory care (dementia/Alzheimer’s), acute pain management, infectious disease care, and respiratory support such as ventilator management.
- Ability to perform clinical skills such as phlebotomy, venipuncture, sterile processing techniques, and IV insertion with confidence.
- Excellent patient assessment skills combined with critical thinking to triage effectively in urgent or emergency situations like emergency room or urgent care environments.
