Victim Services Case Manager

Salary Range: $20hr -$25/hr

Join SSTAR’s Fall River Woman's Center Team as a Case Manager.

The Victim Services Case Manager provides trauma-informed case management, advocacy, crisis intervention, safety planning, and resource coordination to adult victims and survivors of crime. Working within the Women's Center, this position helps survivors achieve safety, stability, and recovery by connecting them with medical, behavioral health, housing, legal, financial, and community resources.

The Case Manager serves as a consistent point of contact, reducing barriers to care while advocating for survivors throughout medical, legal, housing, and social service systems. Services may be provided in the office, community, healthcare settings, and other safe locations as appropriate.

Direct Client Services

• Conduct trauma-informed needs assessments.
• Develop individualized service plans in collaboration with survivors.
• Provide ongoing case management focused on safety, stabilization, and recovery.
• Complete individualized safety planning and regularly update plans based on changing circumstances.
• Respond to referrals in a timely manner and engage survivors using trauma-informed practices.
• Maintain regular contact with survivors to monitor progress toward identified goals.

Advocacy

• Advocate on behalf of survivors with healthcare providers, housing agencies, employers, schools, and community organizations.
• Accompany survivors to medical appointments, housing appointments, and other meetings as appropriate.
• Assist survivors in understanding available community resources.
• Coordinate with Women's Center Team members.

Case Management (build this in first 90 days)

Coordinate referrals and link survivors with:

• Transitional housing
• Medical and behavioral health care
• Substance use treatment
• Financial assistance
• Food and basic needs
• Employment and vocational services
• Transportation
• Childcare resources
• Legal services
• Immigration resources
• Public benefits
• Community support programs

Assist survivors with completing applications and required documentation while following referrals through to resolution whenever possible.

Community Collaboration

• Develop and maintain collaborative relationships with community providers.
• Participate in multidisciplinary case coordination meetings.
• Collaborate with SSTAR programs including Behavioral Health, Health Center, Connect & Protect, SAFEPLAN, Housing, and Recovery Services.
• Represent the Women's Center at community meetings and outreach events.
• Assist with community education and prevention activities.

Documentation and Compliance

• Maintain accurate, timely, and complete documentation within the approved database for data gathering purposes.
• Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, MOVA requirements, and agency policies.
• Participate in supervision, case consultation, and required trainings.
• Maintain knowledge of victim rights, trauma-informed care, community resources, and best practices.

General:

• Complete all required documentation, checklists, and shift communication accurately and on time
• Follow incident reporting procedures and immediately report safety or compliance concerns
• Maintain confidentiality and uphold patient rights, dignity, and privacy at all times
• Attend required trainings, supervision, and meetings
• Maintain awareness of agency communications and policies
• Other duties as assigned

EDUCATION: Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, Counseling, or related field required.

WORK EXPERIENCE:

• One year of experience providing case management, victim advocacy, behavioral health, crisis intervention, or related human services preferred.
• Experience working with survivors of trauma, domestic violence.
• Strong crisis intervention and de-escalation skills.
• Knowledge of community resources and service systems.
• Ability to work independently and as part of a multidisciplinary team.
