The RN-Home Health PRN coordinates and provides skilled nursing care to patients in their homes, focusing on patient advocacy and education.

- Flexibility and Independence - we have a very conservative on call schedule, usually 2 nights in a 6 week period.
- Patient advocacy - enables you to educate patients and families about managing their health.
Job Responsibilities and Requirements:
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- May perform point of care testing according to policies and procedures.
- Collaborates, consults and communicates with physician regarding the plan of care and changes to patient status.
- Credentials include Basic Life Support HealthCare Provider (BLS HCP) and Registered Nurse (RN)
Schedule and Work Environment:
Minimum requirement of 3 eight hour shifts/month. One must be a weekend shift. One Holiday/year. One on call shift/month.
