About this role:
This position is responsible for managing the daily operations of the Care Coordination department, which includes RN Case Managers, Social Workers, and administrative team members. The ideal candidate will have excellent leadership and communication skills, as well as the ability to work effectively with patients, staff, and physicians.
About you:
- You have a Bachelor's degree in nursing or a Master's degree in social work.
- You have at least five years of experience, with two years of progressive leadership.
- You are licensed as a Registered Professional Nurse (RN) or Licensed Clinical Social Worker (LCSW).
Responsibilities:
- Develops and implements specific departmental goals, standards, and objectives that support the strategic plan and vision.
- Assesses quality of patient care and coordinates patient care services with patients, staff, physicians, and other departments.
- Collaborates with others regarding orientation and ongoing education, provides coaching and mentoring of staff, and ensures staff compliance with departmental and organizational policies, procedures, and standard work.
- Monitors departmental budgets, regulatory compliance, departmental contracts, and vendor relations.
- Manages staff relations, including performance management, staff satisfaction, and conflict management.
- Applies the existing body of evidence-based practice and scientific knowledge in healthcare to nursing practice, ensuring that nursing care is delivered based on patient-specific needs and clinical needs as described in the department's Scope of Service.
