Position Summary
The Medical Fiduciary serves as a care manager for individuals under guardianship, overseeing and coordinating medical, mental health, and social services. This role requires strong organization, compassion, and attention to detail to ensure clients’ needs are met while safeguarding their rights and privacy.

Essential Duties & Responsibilities
- Coordinate all aspects of care for individuals under guardianship, including:
- Physician appointments (medical, dental, vision, etc.)
- Nursing care and personal care services
- Therapy services (physical, occupational, speech)
- Psychiatric care
- Educational, social, and vocational services
- Encourage client independence and decision-making while ensuring safety.
- Maintain strict confidentiality and full compliance with HIPAA regulations.
- Communicate with judicial authorities as required and file necessary court documents, including:
- Case information sheets
- Notices of change
- Care plans
- Annual reports
- Communicate appropriately with interested parties while protecting client privacy and adhering to court orders.
- Ensure all documentation and filings are completed accurately and on time.
- Maintain detailed, typed records of client interactions and relevant medical, social, and legal information.
- Identify when guardianship standards are no longer met and petition the court for termination when appropriate.
- Recognize and report mistreatment, abuse, neglect, or exploitation in accordance with mandatory reporter requirements.
- Follow National Guardianship Association standards.
- Perform additional duties as assigned.
Knowledge, Skills & Abilities
- Strong time management skills and ability to meet deadlines
- Proficiency in Microsoft Word and Excel
- Ability to transmit documents via electronic mail
- Excellent communication skills (written and verbal)
- Highly organized and detail-oriented

Qualifications - Required
- High School Diploma or GED
- 3–4 years of relevant experience
- Ability to obtain certification as a Nationally Certified Guardian within 18 months of employment
- Valid driver’s license and proof of car insurance
Qualificiations - Preferred
- Bachelor’s degree in a related field (e.g., social work, criminal justice, nursing, or similar)
- Experience with Medicaid qualification and long-term Medicaid waivers
