What are the responsibilities and job description for the Health Care Provider - Nurse Practitioner or Physician Assistant position at Springfield Health Care Center?
Duties Include:
· Manage acute and chronic illnesses.
· Conduct physical exams and perform/order diagnostic tests and procedures.
· Diagnose and treat health problems.
· Perform patient routine office visits and yearly physicals.
· Prescribe medications as needed.
· Educate health promotion and disease prevention to patients.
Qualifications:
· Licensed as a NP or PA in the state of Ohio.
· MSN or PAC degree from accredited school or University.
· 2 years’ experience. New graduates will be accepted.
· Ability to create a positive work environment.
· Ability to identify and solve problems.
· Friendly and capable of respectfully communicating to both staff and patients.

· Monday through Friday from 8:00am to 5:00pm.
