Responsibilities
- Conduct thorough assessments of patients’ medical, psychological, and social needs utilizing EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems
- Develop and implement personalized care plans aligned with clinical guidelines
- Coordinate with facility's Interdisciplinary Team (IDT)—to ensure effective discharge planning and utilization review
- Monitor patient progress through ongoing communication with healthcare providers, social workers, behavioral health specialists, and family members
- Manage intake processes for new patients requiring complex case management services in post acute care settings
- Ensure compliance with HIPAA regulations by maintaining confidentiality of patient information at all times
- Assist in utilization management by reviewing medical necessity criteria and supporting documentation for appropriate resource allocation
Experience
- Familiarity in discharge planning, utilization review, and managed care processes
- Knowledge of medical terminology and utilization management standards
- Must possess the ability to make independent decisions, follow instructions
- Skilled in navigating EMR/EHR systems while maintaining HIPAA compliance
- Ability to generate written communication that is clear, concise, and well-organized
- Excellent oral communication skills to collaborate effectively across multidisciplinary teams in fast-paced hospital settings
- Experience working in Skilled Nursing (SNF) settings preferred
