Job Summary

Duties
- Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any concerns promptly
- Support patients with specialized care needs including Alzheimer’s care, dementia care, end-of-life care, and memory care
- Perform patient transfers using proper methods and equipment such as Hoyer lifts or patient transfer techniques to ensure safety for both client and caregiver
- Maintain patient confidentiality by adhering to HIPAA regulations and safeguarding sensitive medical records
- Assist with light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe living environment
Experience
- Prior experience working in home healthcare settings or assisted living facilities preferred
- Demonstrated ability to work with geriatric patients and individuals with disabilities or developmental disabilities
- Knowledge of medical terminology, catheter care, wound care, and basic life support (BLS) or CPR certification required
- Experience using EMR (Electronic Medical Records) or EHR systems for documentation preferred
- Familiarity with home health regulations and compliance standards necessary for delivering quality patient care
- Proven ability to communicate effectively with clients and their families while maintaining professionalism and compassion
- Previous roles involving hospice care or heavy lifting are advantageous but not mandatory
