Bilingual Scheduler/Care Coordinator

Job Summary

We are seeking a dynamic and bilingual Scheduler/Care Coordinator to join our team. This role serves as a key point of contact for clients, caregivers, and care managers, ensuring the timely and efficient coordination of homemaker services. The ideal candidate will have excellent communication and organizational skills, strong attention to detail, and the ability to manage scheduling needs in a fast-paced home care environment. This position plays an important role in ensuring clients receive reliable, compassionate, and well-coordinated services.

Duties

• Schedule and coordinate homemaker services based on client needs, authorized hours, and caregiver availability.
• Communicate with clients, caregivers, case managers, and family members regarding service schedules and changes.
• Monitor caregiver schedules, attendance, and service hours to ensure services are provided as authorized.
• Maintain accurate client and caregiver records in the agency’s scheduling and electronic systems.
• Follow up on missed visits, schedule changes, and caregiver coverage needs.
• Address client and caregiver concerns and escalate issues when necessary.
• Ensure services are scheduled and delivered in accordance with agency policies and program requirements.
• Provide professional bilingual communication and customer service.
• Work closely with management and care coordination staff to ensure consistent and reliable homemaker services.
