Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care across diverse healthcare settings. Duties
- Perform detailed patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately while adhering to HIPAA regulations and sterile techniques.
- Assist with complex procedures such as IV infusion, catheterization, wound suturing, and tube feeding, ensuring patient safety and comfort.
- Monitor patients’ conditions closely, including those in ICU or trauma center settings, providing critical support during acute episodes or emergencies.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with conditions such as dementia, Alzheimer's disease, or developmental disabilities.
- Conduct patient education on medication management, disease processes like infectious diseases or pain management strategies, and discharge instructions.
- Maintain meticulous medical documentation of patient care activities, assessments, and interventions in compliance with healthcare standards.
- Support triage processes in urgent care or emergency room environments by prioritizing patient needs based on acuity levels.
Qualifications
- Valid Licensed Practical Nurse (LPN) license from an accredited nursing board.
- Proven experience working in various healthcare settings including hospital medicine, long-term care facilities, assisted living communities, or home & community care environments.
- Knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/ICD-10) for accurate documentation and billing purposes.
- Experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for efficient patient record management.
- Competency in performing phlebotomy, specimen collection/processing, and administering injections with precision.
- Familiarity with specialized areas such as geriatrics, dementia care (including memory care), hospice care, pediatric nursing (including neonatal ventilator management), or trauma center protocols is highly desirable.
- Strong critical thinking skills with the ability to perform comprehensive patient assessments and respond swiftly to emergent situations like airway management or acute pain episodes.
- Excellent communication skills to work effectively with diverse populations including children with developmental disabilities or elderly patients requiring complex case management.
