Schedule: Per Diem / On Call | Weekdays, 4:00 PM–12:00 AM, and weekends as needed
Pay Rate: $55.00–$60.00 per hour
Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and compassionate Registered Nurse (RN) Supervisor to provide clinical leadership and oversee nursing operations on a per diem, on-call basis. Coverage includes weekday evening shifts from 4:00 PM to midnight and weekends as needed.

Key ResponsibilitiesOversee and coordinate resident care during assigned shifts.
Supervise nursing staff and provide clinical guidance and support.
Monitor resident conditions and respond promptly to changes in health status.
Collaborate with physicians and interdisciplinary team members to support individualized care plans.
Ensure compliance with facility policies, procedures, and applicable regulatory standards.
Promote resident safety, quality care, and respectful communication with residents and families.
Provide hands-on nursing care and clinical support as needed.
Address nursing concerns and assist staff with problem-solving.
Ensure accurate documentation and effective communication between shifts.
Active New York State Registered Nurse (RN) license.
Minimum of two years of nursing experience.
Previous supervisory or charge nurse experience preferred.
Skilled nursing or long-term care experience preferred.
Strong clinical judgment, leadership, communication, and organizational skills.
