Job Summary:

Spring Creek Healthcare Center, a premier 179-bed nursing home located in Perth Amboy, NJ, is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. As a Social Worker at Spring Creek, you will play a vital role in providing exceptional care to our patients and residents, while promoting their physical, emotional, and social well-being.

Responsibilities:

• Conduct initial and ongoing assessments of patients' and residents' social, emotional, and psychological needs
• Develop and implement individualized care plans to address identified needs and goals
• Collaborate with the interdisciplinary team to ensure comprehensive care and promote positive outcomes
• Provide counseling and support to patients, residents, and their families to address grief, loss, and other emotional challenges
• Facilitate communication between patients, residents, and healthcare providers to ensure timely and effective care
• Identify and address potential barriers to care, such as cultural or linguistic differences
• Participate in quality improvement initiatives to enhance patient and resident care and satisfaction
• Maintain accurate and detailed records of patient and resident interactions and outcomes
