At Spring Creek Healthcare Center, we are seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our team of caring professionals. As a key member of our nursing staff, you will provide exceptional patient care and contribute to the overall well-being of our residents.
Responsibilities:
- Provide direct patient care under the supervision of the Registered Nurse, including administering medications, performing vital signs, and completing treatments as ordered
- Develop and implement individualized care plans in collaboration with the interdisciplinary care team
- Maintain accurate and up-to-date patient records, including progress notes and care plans
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Communicate effectively with patients, families, and healthcare team members to ensure a smooth delivery of care
- Collaborate with other disciplines to develop and implement effective care strategies
- Provide emotional support and comfort to patients and families in a caring and compassionate manner
- Current LPN/LVN license in the state of New Jersey
- 0 years of experience (new graduates welcome to apply!)
- Ability to work in a fast-paced environment with frequent interruptions
- Strong communication and interpersonal skills
- Ability to lift, push, and pull up to 50 pounds
- Ability to work varying shifts, including days, evenings, and weekends
