What are the responsibilities and job description for the Registered Dietitian (RD) position at Split Rock Rehabilitation and Healthcare Center?
Duties and Responsibilities of the Registered Dietitian:
- Working with interdisciplinary team members to ensure optimal care is provided to all residents
- Assess, Develop and implement nutrition care plans to facilitate the provision of high quality and appropriate nutrition care to residents.
- Participate in interdisciplinary care team meetings
- Sigma Electronic assessments, care plans, MDS’s, progress notes, weight monitoring
- Geri Menu
- Provide nutrition counseling to residents and family, make diet & supplement recommendations
- Knowledge of diets and tube feeding
- Meal Observation
- Must have experience as a Registered Dietitian
- Must have experience in long term care
- Must have EMR (sigma care, PCC) experience
- Candidate must possess good verbal, writing, and organizational skills.
- Candidate must have strong computer knowledge
- Full Time Position
Salary : $45 - $55
