Home Health Aide (HHA) - Resident Transport & Support Services

Split Rock Rehabilitation & Health Care Center is seeking compassionate and dependable Home Health Aides (HHAs) to assist residents with transportation and non-clinical support services throughout the facility. This role is ideal for individuals who enjoy working with seniors and helping residents maintain their comfort, dignity, and quality of life.

Responsibilities

• Safely transport residents to and from therapy, activities, dining rooms, appointments, and other areas of the facility.
• Assist residents with wheelchairs, stretchers, and other mobility equipment.
• Provide support with resident comfort and non-clinical needs during transport.
• Escort residents to activities and appointments as directed.
• Observe and report any changes in resident condition or concerns to nursing staff.
• Maintain a courteous, professional, and resident-centered approach at all times.
• Follow facility policies regarding safety, infection control, and resident rights.
• Assist with keeping transportation equipment clean and in good working order.
• Support nursing and interdisciplinary teams as needed.
  
  
  

Qualifications

• Current New York State Home Health Aide (HHA) certification required.
• Previous experience in a nursing home, rehabilitation center, or healthcare setting preferred.
• Ability to safely transport and assist residents with mobility needs.
• Strong communication and interpersonal skills.
• Reliable, compassionate, and team-oriented.
• Ability to work flexible schedules, including weekends and holidays as needed.
