Responsibilities:
- Admit and reassess home health patients
- Coordinate and supervise Home Health Aides
- Conduct home care visits and reassess regularly
- Implement care plans and document progress
- Educate patients and caregivers
Requirements:
- NYS RN License
- Positive collaboration skills
- Strong communication and time-management
- Ability to travel daily
- COVID-19 vaccination required
Language Skills: Bilingual in English & Spanish (preffered)
Certifications:
- BLS Certification
- RN License
Location: On the road
