Physical Therapist (PT) – Home Health

Location: West Chester Township, OH 45069 (Travel within surrounding communities required)

About the Role
Spirit Homecare & Hospice is seeking a licensed Physical Therapist (PT) to provide in-home therapy services for patients recovering in their homes. You will evaluate, treat, and support patients to improve mobility, independence, and quality of life. This role offers a flexible schedule and collaboration with a dedicated interdisciplinary care team.
Compensation & Schedule
- Start of Care / Resumption Visits: $100 per visit (includes travel reimbursement)
- Routine Visits: $55 per visit (includes travel reimbursement)
- Flexible scheduling: Full-time or PRN assignments
- Work alongside RNs, PTAs, and other healthcare professionals

Key Responsibilities
- Conduct physical therapy evaluations, including OASIS assessments
- Develop and implement individualized treatment plans in coordination with the care team
- Deliver therapy to improve mobility, balance, and safety in patients’ homes
- Maintain accurate documentation per agency and Medicare standards
- Collaborate with RNs, PTAs, physicians, and other healthcare professionals
- Supervise and guide PTAs as needed
- Educate patients and caregivers on exercises, mobility, and home safety
Qualifications
- Active Ohio Physical Therapist license
- Experience in home health preferred; SNF or outpatient experience also considered
- Familiarity with OASIS and Medicare documentation preferred
- Strong clinical, communication, and time-management skills
- Valid driver’s license, reliable transportation, and proof of auto insurance
Schedule & Service Area
- Flexible hours, Monday–Friday, based on caseload
- Travel within West Chester Township and surrounding communities
