We are looking for a licensed Physical Therapist (PT) to join our growing home health team and help patients regain strength, mobility, and independence from the comfort of their homes.

• Start of Care (SOC) / Resumption of Care (ROC): $100 per visit
• (Includes $90 base $10 mileage reimbursement)

Clinical Evaluation & Care Planning

• Perform comprehensive PT evaluations, including OASIS assessments
• Develop, implement, and update individualized treatment plans
• Deliver goal-directed therapy to restore physical function and safety
• Ensure timely, accurate documentation in accordance with Medicare and agency standards

Team Collaboration

• Communicate and coordinate with RNs, PTAs, physicians, and other clinicians
• Supervise PTAs and provide clinical oversight when needed
• Participate in case conferences and care planning meetings

Patient & Family Education

• Instruct patients and caregivers on home exercise programs, fall prevention, and mobility techniques
• Promote safe and independent living through therapeutic guidance

Qualifications:

• Active Physical Therapist (PT) license in Ohio (required)
• Home health experience preferred, but SNF or outpatient backgrounds welcomed
• Familiarity with OASIS documentation and Medicare regulations (preferred)
• Excellent communication, organization, and clinical judgment
• Valid driver’s license, reliable transportation, and auto insurance

Work Schedule & Service Area:

• Schedule: Monday to Friday | Flexible based on patient visits
• Territory: West Chester Township and surrounding communities
• Setting: Field-based | Travel required between patient homes
