SpineSearch is partnering with a leading healthcare system in the NY Metro area to identify a Certified Diabetes Care and Education Specialist (CDCES) to join a mission-driven multidisciplinary team serving diverse patient populations across pediatric, adult, and geriatric care settings.
The Certified Diabetes Educator will play a vital role in bridging the gap between clinical care and patient self-management for individuals with prediabetes, type 1 diabetes, and type 2 diabetes. This position focuses on delivering high-quality Diabetes Self-Management Education and Support (DSMES), disease prevention counseling, and medical nutrition therapy while collaborating closely with providers and care teams to improve patient outcomes.
Key Responsibilities:
• Provide individualized and group diabetes education and nutritional counseling for patients with diabetes, prediabetes, and other chronic conditions
• Develop personalized care plans that address dietary, cultural, socioeconomic, and health literacy needs
• Conduct comprehensive assessments, monitor patient progress, and adjust interventions as appropriate
• Educate patients on diabetes technologies including CGMs and insulin pumps
• Collaborate with endocrinologists, primary care providers, nephrologists, cardiologists, and interdisciplinary care teams
• Promote adherence to ADCES 7 self-care behaviors and evidence-based diabetes management practices
• Conduct Medical Nutrition Therapy (MNT) visits for patients with CKD, CHF, hypertension, obesity, hyperlipidemia, and related conditions
• Participate in quality improvement initiatives and accreditation compliance efforts
• Advocate for equitable access to diabetes education and nutrition services for underserved communities
Qualifications:
• Bachelor’s Degree in Food, Nutrition, Dietetics, or Nursing
• Certified Diabetes Care and Education Specialist (CDCES) required
• Registered Dietitian Nutritionist (RDN) required
• Minimum 2 years of experience as a Registered Dietitian, preferably with pediatric and adolescent counseling experience
• Current BCLS certification required
