Nurse Practitioner – Skilled Nursing Facility (SNF) / Rehab & Pain Management
Spine Pain and Rehab Associates is a growing rehabilitation and pain management practice serving the Kansas City metropolitan area. Our team consists of three physicians and ten nurse practitioners providing rehabilitation consultations, pain management, and post-acute care services throughout Missouri and Kansas.
We are seeking a motivated and compassionate Nurse Practitioner to provide care in skilled nursing facilities throughout the Kansas City metro area, including facilities in Blue Springs and surrounding communities. This position focuses on rehabilitation medicine, pain management, and post-acute patient care in the SNF setting.
The ideal candidate is clinically sharp, eager to learn, and enjoys working closely with physicians, therapists, nursing staff, and case managers to improve patient outcomes.
Responsibilities
- Perform comprehensive patient evaluations and follow-up visits in skilled nursing facilities.
- Manage pain and rehabilitation-related conditions for post-acute and long-term care patients.
- Develop and implement treatment plans in collaboration with supervising physicians.
- Evaluate functional status and support rehabilitation goals.
- Collaborate closely with physical therapists, occupational therapists, speech therapists, nursing staff, and case managers.
- Prescribe medications and order diagnostic studies as appropriate.
- Monitor patient progress and adjust treatment plans accordingly.
- Provide patient and family education regarding pain management, rehabilitation, and recovery expectations.
- Complete accurate and timely EMR documentation.
- Travel to assigned skilled nursing facilities within the Kansas City metro area as needed.
Qualifications
- Active Nurse Practitioner license in both Missouri and Kansas, or ability to obtain dual licensure.
- DEA license or eligibility to obtain one.
- Previous experience in skilled nursing, rehabilitation, pain management, hospital medicine, geriatrics, or post-acute care is preferred but not required.
- Strong clinical judgment, communication, and organizational skills.
- Ability to work independently while maintaining close collaboration with physicians and facility staff.
- Reliable transportation and willingness to travel within the Kansas City metro area.

Pay: $110,000.00 - $120,000.00 per year
