Overall Function:
Conducts level of care assessments and coordinates referral and transportation, if needed, to the recommended treatment services. Will also link both adults and adolescents to ancillary services as needed to support access to treatment services and recovery.
Essential Functions:
- This position is a dual position providing both adolescent and adult services by providing case management, intervention, and level of care assessments to adolescents onsite at the schools within Westmoreland County AND to adults within an office and community setting.
- For Adolescent School-based services, travels to all the school districts within Westmoreland County. For Adult Community services, conducts services within the office and community setting.
- Coordinates and maintains contacts and relationships with referral sources, which would include, but not limited to, school-based personnel at all school districts within Westmoreland County, school-based liaisons, and community agency referral sources within the county.
- Attends and represents the agency/program at various training sessions and community education presentations.
- Presents professional educational seminars as deemed necessary to key school related individuals and treatment teams.
- Participates in all appropriate clinical staff meetings and community meetings and maintains beneficial clinical dialogue with all members of the treatment team and identified school personnel regarding assigned clients.
- Participates in the staff 24/7 on-call after hours rotation process to assist clients, telephonically, with assessing higher levels of treatment care.
- Assesses Social Determinants of Health needs and assists in addressing those needs which may be a barrier to treatment and recovery.
- Initiates appropriate Intra and Inter Agency contact to facilitate the care of the individuals that are served.
- Maintains current client case record activity and progress; prepares and submits all required program reports and documentation in an accurate and timely manner.
- Other duties as assigned or needed for the program and services.
- Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.
Qualifications:
- Master’s degree in social work, Psychology, Sociology, Nursing, Chemical Dependency, Criminal Justice or related social service fields from an accredited college or university, OR a master’s degree in an unrelated field with a minimum of 2 years work experience in a related social services field.
- Must be available to participate in a rotation on call after hours telephonic linkage and referral process/
- Demonstrated knowledge of mental health programs and available social services.
- Demonstrated ability to obtain vital information during an interview to make proper service recommendations.
- Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
- Must successfully obtain Criminal History Clearance from the PA State Police, Child Abuse Clearance from the PA Department of Human Services and FBI Fingerprint Clearances.
- Ability to observe and recognize verbal and non-verbal signs of physical or mental disorders.
- Ability to transport oneself in an insured vehicle, as program needs dictate.
- Ability to detect and identify violations pertaining to program regulations.
