Provide skilled home health nursing care to geriatric patients across multiple communities. This role delivers skilled nursing visits and chronic disease management under the clinical supervision of a Registered Nurse case manager, supporting patients and families through compassionate care, education, and coordinated services.
Responsibilities
- Conduct patient assessments and monitor vital signs during home visits.
- Administer medications and provide IV care in accordance with clinical orders.
- Deliver wound care and support infection control practices.
- Develop and implement individualized care plans for geriatric patients.
- Provide patient and family education, caregiver communication, and family support.
- Coordinate care with the RN case manager, physicians, caregivers, and interdisciplinary teams.
- Complete daily electronic charting and clinical documentation in AXXESS EMR.
- Participate in interdisciplinary care conferences and manage physician orders.
Required Qualifications
- Five or more years of experience in home health nursing.
- Completion of an accredited Licensed Practical Nursing program.
- Active Licensed Practical Nurse (LPN) license.
- Basic Life Support (BLS) certification.
- Proficiency in medication administration, wound care, and patient assessment.
- Knowledge of electronic health records, infection control, and care planning.
- Strong clinical documentation, patient education, and communication skills.
