Registered Nurse Hospice Case Manager – Field Position

$53 - $56 hourly

• Provide compassionate, hands-on nursing care to hospice patients in their homes, ensuring comfort and dignity.
• Manage symptoms effectively, tailoring care plans to meet the unique needs of each patient and family.
• Offer emotional support and guidance, fostering a trusting relationship with patients and their loved ones.
• Collaborate with an interdisciplinary team, sharing insights and expertise to enhance patient care.
• Conduct regular assessments and adjust care plans as needed, ensuring optimal patient outcomes.
• Educate patients and families on hospice care processes, empowering them with knowledge and confidence.
• Maintain accurate and timely documentation, reflecting the quality and continuity of care provided.

• Must be willing to drive
• Active Registered Nurse (RN) license in New Jersey
• 2–3 years of clinical nursing experience (hospice, palliative care, or related field preferred)
• Knowledge of hospice philosophy and end-of-life care
• Strong communication and interpersonal skills for working with patients, families, and team members
• Ability to work independently and as part of an interdisciplinary team
• Excellent time management, organization, and attention to detail
• Comfortable using electronic health records (EHR) and related healthcare software
