Role Summary
The Support Coordinator serves as an essential member of the Care Management Program, responsible for coordinating services for adults with developmental disabilities. This position involves managing a caseload, maintaining detailed oversight of service delivery, and ensuring that each individual receives support tailored to their personal goals and needs.
In this role, you will develop individualized care plans, engage with families and providers, and ensure that services remain aligned with the client’s preferences. Regular in-person and phone-based check-ins are required, along with flexibility to adjust to client schedules and circumstances.
Primary Duties
- Create individualized, person-centered service plans
- Guide clients and their families through available community resources and supports
- Complete quarterly home visits and conduct monthly follow-up calls
- Monitor all services on an ongoing basis to ensure quality and compliance
- Maintain accurate documentation and case records
- Work closely with internal and external care partners to address any changes in needs or services
- Comply with all state regulations and internal policies related to care management
Qualifications
- Bachelor’s degree or higher in Social Work or a related discipline
- At least one year of case management experience and one year of experience supporting individuals with developmental disabilities
- Relevant experience may include paid work, volunteer roles, or caregiving
- Experience coordinating services for other populations, where some individuals had developmental disabilities, will be considered
- Valid New Jersey driver’s license, dependable personal transportation, and a willingness to travel
- Strong organizational skills, self-direction, and attention to detail
- Proficiency with Microsoft Office, Adobe Acrobat Pro, and standard office equipment
- Access to a functional home workspace, including a laptop, printer, cell phone, WiFi, and a hotspot
