Title: Senior LTSS Service Care Manager (RN)
Location: Laredo, TX | Hybrid
6-month contract with a strong chance to extend or convert
Schedule: Monday–Friday, 8 AM–5 PM (OT as needed)
Required: Registered Nurse
Role Overview:
This role focuses on in-home assessments for pediatric members in the Laredo area. The nurse spends 3 days a week visiting members’ homes and 2 days on documentation and follow-ups, ensuring all state and company compliance requirements are met.
Core Responsibilities
- Conduct face-to-face pediatric assessments in members’ homes
- Complete and submit assessments, forms, and documentation on time
- Perform member outreach and care coordination activities
- Follow LTSS, state Medicaid, and internal compliance guidelines
- Balance fieldwork with documentation and administrative tasks
Must-Have Requirements (Non-Negotiable)
- Active RN license
- Pediatric experience (required)
- Case management exposure
- Comfort with field-based/home visit work

- Home Health experience (strongly preferred)
