Key Responsibilities:
- Conduct comprehensive initial assessments and routine evaluations of residents' health statuses.
- Craft personalized care plans to address the unique needs of each resident, emphasizing preventive care and health maintenance.
- Oversee and coordinate ongoing care for residents with chronic conditions like diabetes, hypertension, and heart disease.
- Prescribe appropriate medications and treatments, adhering to facility protocols and regulatory guidelines.
- Collaborate with a dedicated interdisciplinary team of physicians, nurses, therapists, and caregivers to ensure coordinated, holistic care.
- Educate and support residents and their families in health management, disease prevention, and lifestyle adaptations.
- Respond swiftly to acute medical needs and emergencies, ensuring timely intervention and stabilization.
- Maintain precise and current medical records/documentation for all residents.
- Engage in regular team meetings, case conferences, and continuing education to remain up-to-date with best practices and industry standards.

Qualifications:
- Current unrestricted Nurse Practitioner license in New York.
- Proven proficiency in conducting comprehensive health assessments and managing chronic conditions.
- Exceptional interpersonal and communication skills, with the ability to build rapport with residents, families, and staff.
