Requirements
- Active Indiana Pharmacist license in good standing
- Prior inpatient or hospital pharmacy experience preferred
- EPIC experience required
- Experience with medication reconciliation and discharge counseling
- Strong clinical judgment, communication skills, and attention to detail
- Verify, prepare, and dispense medications in accordance with state and federal regulations
- Support medication reconciliation and discharge medication counseling
- Monitor drug therapy for dosing accuracy, interactions, and contraindications
- Participate in antimicrobial stewardship and pharmacokinetic dosing support
- Maintain accurate documentation and controlled substance accountability
- Support pharmacy safety, quality, and regulatory compliance efforts
- Collaborate with interdisciplinary clinical teams
- Maintain patient confidentiality and professional standards
