Job Summary:
We are seeking compassionate Registered Nurses (RNs) to join our team as Case Managers, working one-on-one with patients in their home environment. As a Case Manager, you will establish and oversee all aspects of patient care, creating a Plan of Care and following physician orders to achieve outcomes of increased patient independence and satisfaction. Responsibilities:
Assume responsibility for assigned patient's care from admission through discharge, effectively executing patient's Plan of Care
Collaborate with patients, family/caregiver(s), ordering physicians, other care team members, and community resources
Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes
Participate in case conferences with other clinicians, reviewing services, clinical documentation, and discussing pertinent patient care issues and goals
Participate in on-call rotation as reasonably assigned
Qualifications:
Minimum of 2 years of nursing experience, with a preference for 2 years in an acute or post-acute care setting
Active State Registered Nurse licensure required
Proof of current CPR certification and valid Driver's license
